Texas Health Fort Worth is in search of a PRN Social Worker (Job Id#21191) to join the Case Management Team.
Schedule / Hours: PRN / As Needed
Minimum Requirements / Qualifications:
- Bachelor\'s degree in Social Work
- Licensed by the Texas State Board of Social Work Examiners
- Minimum of Three (3) years of Social Work experience
- Fundamental knowledge and skills in Social Work assessment and intervention as well as alternative levels of care, community resources and healthcare industry
- Sound interpersonal skills and able to multi-task and think critically in a fast-paced environment
- Independently practices as part of the healthcare team
Preferred Requirements / Qualifications:
- At least Three (3) years of Healthcare related Social Work
Duties / Essential Responsibilities:
- Obtains accurate and pertinent information on patient/family social, psychological, environmental and financial factors affecting the medical situation and recovery and also overall functioning
- Initiate actions of active listening and reflective skills to build therapeutic relationships to facilitate a cognitive or behavior change
- Make appropriate interventions and referrals in addressing psychiatric/ mental health aspects of the care
- Explore and address substance use/ abuse/ dependence in the aspects of the care and makes appropriate referrals
- Apply crisis intervention and brief therapy theories, models and techniques of counseling
- Refer patients to internal or external resources to provide higher level or continuity of care
- Use research/evidenced based practice for treatment planning
- Maintain knowledge of alternative levels of care and other healthcare providers for post discharge
- Educate patient/proxy of available options of alternative levels of care
- Coordinate seamless transitions across the continuum
- Develop and update list of area healthcare service providers (HH agencies, DME companies, Rehabs, LTACs, SNUs, ICFs, Hospice agencies, assisted livings, adoption agencies)
- Update and maintain lists of network providers for frequently used payors
- Network with peers to problem solve on complex cases
- Education, interactions and plans reflect age-specific needs
- Mutually develop discharge plan including back up plan
- Obtain authorizations for transfers to alternative level of care and needed transportation
- Send relevant and sufficient medical record information to receiving provider to foster continuity of care
- Correctly facilitate all forms related to transfers of care (MOT/Consent, NH transfer form, ambulance transfer form, patient choice form, title XIX form, adoption forms)
- Collaborate with patient/proxy and healthcare team to procure placement and coordinate transitions based on clinical need, level of care and payor limitations
- Prioritize referrals in a timely manner to prevent delays, denied days or avoidable days
- Inform patient/proxy of projected timing of transfer when known
- Facilitate case conferences on complex cases
- Develop and maintain effective working relationships with internal and external resources
